Interdisciplinary cooperation in planning care for clients with multiple sclerosis in residential respite service

The diploma thesis deals with the effectiveness of interdisciplinary cooperation by setting up processes focused on communication and information sharing in a multidisciplinary team. It is based on theoretical starting points mainly in connection with a systemic approach to the client. The principle of case management, individual cooperation with the client and the consulting paradigm of social work were applied here. The aim of the work is to identify possible forms of effective interdisciplinary cooperation in the selected facility, using the example of which it presents the elements affecting the quality of outputs of care planning for clients with multiple sclerosis in the conditions of a specific residential respite service. In the theoretical part of the thesis, a unique organization in the Czech Republic, which has been involved in the care of people in a long term, especially in the advanced stage of the disease and which includes a residential respite service, is presented. Specifics of Individual Planning of Diakonie ČCE Center Rolnička

The topic of this bachelor thesis is the process of individual planning in social services. The main aim of my bachelor thesis is to find out how the individual planning is carried out in Diakonia ČCE, in the Rolnička centre, in social services in Tábor. With regard to the main objective of the thesis, I have chosen three research questions. Who is involved in the preparation of the individual plan of the client? What experience do key workers have with the individual planning process? The empirical section describes the research for which a qualitative research strategy was chosen. The data required for the research was collected through semi-structured interviews. The research sample consisted of ten informants from among the key workers of the individual social services of the Rolnička centre in Tábor. Subsequent data analysis was conducted using Interpretative Phenomenological Analysis. Five themes emerged from this analysis. These are: the key worker's experience of the individual planning process at his/her workplace, the importance of the training course for the course of individual planning, how the key worker perceives the client's autonomy during individual planning, how the key worker perceives the client during individual planning, and the key worker's relationship with his/her key clients. The research showed that the method of person-centred individual planning called Map is used in the social services of the Rolnička centre in Tabor. The themes of the IPA analysis then led to the answers to the remaining two research questions. The client, the client's key worker, the social worker, the head of the social service and in some cases the client's parents or guardians are mainly involved in drawing up the individual plan. Key workers then described their relationships with their key clients, their perceptions of the client's autonomy, or the client's feelings during the individual planning process. Training in individual planning was also significant from their perspective. The information obtained from the research of this bachelor thesis could serve as a basis for supplementing the existing methodologies of individual planning of social services in the Rolnička centre in Tábor.
Cooperation between health care staff and social workers in retirement home

The subject of this bachelor thesis is discussing cooperation between health care and social care professionals in nursing homes. The aim of this thesis is to show the meaning and importance of this symbiosis for making sure that all clients' needs are met. The work is structured into 4 chapters. First chapter is a brief characteristics of the specifics of senior age. Second chapter then describes care homes, their mission and what they focus on. The categories of different social and health care workers usually found in care homes are discussed in the third chapter. The last and the most important part deals with the cooperation between the professionals. This part is complemented by experience examples which then show the importance and meaning of the cooperation between above mentioned health and social care professionals.

The diploma thesis deals with the issue of the process of individual planning of social services in a residential facility for the elderly with the aim of identify the roles of a social worker and ethically problematic areas in the planning social services in a residential facility for the elderly. The text of the thesis is divided into three thematic headings. The first thematic heading is devoted to a general view of the process of individual planning in a residential facility for the elderly. The second thematic heading deals with presenting the roles of a social worker that he or she can perform during the individual planning proces. It also discusses ethical aspects in residential facility for the elderly. The final thematic heading presents possible problematic situations that may occur during the process of individual planning in a residential facility for the elderly, which are supported by practical knowledge based on a research probe.

Providing Care Services in the Districts of Municipalities with Extended Competence of the City of Benešov

The diploma thesis deals with the provision of care services in the Districts of Municipalities with Extended Competence (DMEC) of the City of Benešov. Its aim is to describe the care service in DMEC Benešov, ways of its provision and changes related to the ongoing transformation, from the perspective of providers and from the perspective of clients. Another goal is to evaluate the expansion of the field care service onto weekends and into rural areas in DMEC Benešov. The thesis is divided into theoretical and practical part. The theoretical part deals with the issue of care services. The practical part examines the area of the issue and is aimed to find answers to two main research questions: How is the care service provided in DMEC Benešov? How is the transformation of the Care Service of the Benešov District? A qualitative research strategy, a method of questioning and a semi-structured interview technique were chosen. The research group consisted of 12 informants, who were selected through deliberate selection, purposeful selection technique and their availability. The results were processed by data analysis using grounded theory. From the research, it can be concluded that the care service in DMEC Benešov has a stable team that supports clients to stay at home. The care service is mostly provided to seniors, who most often receive services such as lunch delivery, taking care of the household and assistance with personal hygiene. The care service provides a service on weekends even in rural areas, where it covers the so-called white spaces. Thanks to the project Care Service - Opportunity for a Life at Home II. The Care Service of the Benešov District managed to focus on professional work, expand the target group with families with a child/children and extend the operational hours until 19:00 and on weekends. However, the research revealed that in the DMEC Benešov there is not much interest in providing care services during weekends. The results can be used as feedback for care service providers in DMEC Benešov and as examples of good practice for other care service providers.
Identification of the extent of necessary support provided to children and adults with physical and combined disabilities

The bachelor thesis is focused on identifying the extent of the necessary support provided to children and adults with physical and combined disabilities within day care centres and week care centres. The aim of the work is to find out how the necessary support is implemented for these users, to find out which methods are used by social service providers to determine the extent of the support, how the necessary support is transformed into individual planning, and how individual planning works within the regular practice of the social worker. The theoretical part of the thesis describes physical and combined disabilities, defines the work of the social worker and social services, evaluation and overview of the necessary support provided to children and adults with physical and combined disabilities and it also contains chapters focused on individual planning. In the practical part of the bachelor thesis, a qualitative research survey using the method of semi-structured interview has been carried out, and a sample of respondents from the social workers working in day or week care centres in the Czech Republic have been approached. Research questions on methods of identifying the extent of the support needed for children or adults with physical and combined disabilities and methods of using these findings in the individual planning of the course of the social service have been identified. Respondents' responses have been processed by the open coding methodology.
Individual planning in NZDM

The main topic of this diploma thesis is individual planning in NZDM from the perspective of employees who make IP. This thesis has a theoretical and a practical part. The theoretical part explains in particular the basic concepts related to the topic. The practical part is focused on research formed from interviews with social workers working in the established NZDM. I chose qualitative research with the help of semi-structured interviews for my research. I analyzed obtained data using open and axial coding. Then individual categories were created among which I was looking for relationships based on axial coding. Then I created a conceptual model that helped focus on data more systematically.
Activation Activities and Therapies in the Nursing Homes with Special Regime

The Bachelor's work focuses on providing activation activities and therapies in nursing homes with a special regimen. The aim of Bachelor's thesis is to map out the provision of activation activities and therapies to users at home with a special regimen. The work is divided into theoretical and practical parts. The theoretical section is divided into two main chapters and nine sub-chapters. The first chapter defines the main terms from a home environment with a special regimen - target group, workers of a home with a special regimen and their education, activism activities and individual planning. The second chapter of the theoretical section explains the activism activities and therapies which are provided the most at homes with a specific regimen. The qualitative research, questioning method and semi-structured interview technique are used to collect data for the practical part. The HVO survey question has been determined: How activation activities and therapy are provided at home with a special regimen. In addition, there are determined two sub-research questions DVO1: What is the education of workers who providing activation activities and therapy at home with a special regimen? DVO2: How do homes with a special regimen co-operate with special educators? The research was realized during the month of February and March 2020 at homes with a special regimen in the South Bohemia. The results follow that activation activities and therapies at homes with a special regimen in the South Bohemia are provided in a broad range, individual and group forms. The workers who procure the activation continuously supplement their education and they educate in new areas. The results of the bachelor's thesis are useful for students of selected fields, for workers who providing activation activities and therapies at homes with specific regimen and as feedback for the facilities where the research is realized.
Formal Caregivers in Retirement Homes and Their Role in Fulfilling Wishes of the Elderly

The bachelor thesis focuses on formal caregivers in retirement homes and their role in fulfilling the wishes of the elderly. It describes the system of individual planning and presents its concrete form on the example of a home for the elderly established by the Central Bohemia Region. The case study uses semi-structured interviews with seven formal caregivers of the retirement home and an internal methodology of individual planning. The work captures the attitudes of the caregivers towards individual planning and the ways in which they deal with this task. The analysis demonstrates the importance of collaboration between the home's staff and the role of the individual planning coordinator in managing key workers. Carers' attitudes to individual planning can be classified on a spectrum of subjective rejection/acceptance. This rejection/acceptance is related to perceptions of the benefits and appropriateness of the current form of individual planning. The main problems faced by formal caregivers in individual planning are identified. These include lack of clarity about individual planning and a lack of understanding of its importance not only by caregivers but also by the clients.
